How to Block Spam from Google Forms
Gmail filters can assist you in preventing spam emails from Google Forms from arriving in your mailbox.
The sender’s email address appears in all pre-filled Google Forms emails as follows:

You can create a filter in Gmail that will automatically delete emails that have Google Forms as the sender.
You can also access the form link from the email and click the “Report Abuse” option to report the form to Google. However, that is unlikely to be a successful technique because spammers may always switch to an other Google account.
